FINAL FORMS - NEW OPTION FOR PARENTS!
As a reminder, every athlete must have a current physical  prior to trying out for a sport. Effective this spring,  physical forms may be uploaded in your athlete's profile and you may keep the original physical form.  Final Forms registration is open and parents should update their information.    Please make sure you check the sport your athlete is participating in and sign all of the required forms.  Parents of 7th grade students and new students to WMS or WMHS must register their athlete(s) on final forms.  In accordance with The Ohio High School Athletic Association, physicals are now good for 13 months instead of 12 months.  As physicals are entered into final forms, the 13 month time period will be entered.

Licking Memorial Health Systems (LMHS) announces the 2022 Sports Screening Program for the Licking County Community. The program will offer screenings to all student athletes and band members, entering grades 7 through 12, required to comply with the Ohio High School Athletic Association sports physical pre-participation requirements. Students entering grades 11 and 12 in specific educational training programs at CTEC will also be eligible.

The program will include sports physicals, heart screenings, and baseline concussion testing. Heart screenings will be provided, according to LMHS program guidelines for students going into grades 7, 9, and 11, as part of medical clearance for the pre-participation physical.   ImPACT baseline concussion screenings will be an option available for athletes that play contact sports and do not have concussion screening available through their high school. Students who may have already completed a physical through their school are still able to register for just the heart screenings and/or ImPACT testing, if it is not available through the school and they would like to have these screenings done.

Registration for the program is open to the community on the LMHS website beginning April 1, 2022. Parents/students can select a date from the calendar of available sessions to sign up for the day that will work for their individual schedules. The program is free to all participants; however, registration is required to participate. Please click on the link to register: [email protected] The program dates will begin May 3rd and will end May 31st, with the first two dates located at the Pataskala Health Campus. Each scheduled night will be open to the first 90 individuals to register. Please visit the LMHS website for additional registration details.  Once an individual registers for the program they will be linked to the packet of LMHS forms required to be complete prior to their appointment and the OHSAA pre-participation physical form. If your school utilizes a physical form other than what is provided by OHSAA (Priv-IT, Final forms, etc.), please have your athletes complete that form in advance to bring with them the night of their appointment; otherwise, they will need to complete the OSHAA form prior to check in. The program will be once again administered this year with social distance practices and masks required for attendance. Please let me know if you have any questions.
