Thankfully, we have a streamlined paperless enrollment process for all parents/legal guardians through our online portal to get you started.

Kindergarten Registration and Screening
Each spring, students that will be 5 years old on or before August 1 of the current calendar year are eligible to register for the upcoming school year, and a legal guardian can complete early registration through April to secure an appointment at Kindergarten Screening in May! You can anticipate spending 60-90 minutes at Screening, as students will work with teachers to get some baseline data on what your child already knows while parents/guardians have an informational session with School Administrators. This data allows our teachers to begin planning for your child’s individual needs, come fall!
K–12 Registration
We are excited to announce that Southwest Licking School District is expanding services with FinalForms!
FinalForms is an online forms and data management system that guarantees compliance, distributes data and reduces liability within our district. FinalForms saves student data from sport-to-sport, club-to-club, year-to-year, eliminating paperwork from your life! At minimum, you will log in to update, verify and sign forms once per year, or as often as your information changes.
Step One - Online Pre-Registration
Click HERE to complete the online pre-registration from any device if your student RESIDES WITHIN the SWL Boundaries. (Guide to help with registration) Once you have created your account and completed all questions within the online forms, you will be instructed to upload the required documentation from the detailed checklist provided to you within the application. If you have any questions about the pre-registration or checklist, please call 740-927-3941 during our normal business hours.
Step Two – Enrollment Appointment
You will select and confirm an appointment date and time for each student to bring your documents to the Registrar. Appointments for K-12 registrations will be at our District Office, located at 927-A South Street in Pataskala. Your child is welcome to come, but it is not required; please allow 15-30 minutes to review your documents. ALL required documentation should be uploaded to the final forms account. Please be sure to bring photo ID along with a copy of the original birth certificate for review at your appointment. The Preschool Registration section below outlines information on registration for that program.
Items to upload:
- Parent/guardian current Photo I.D. ( also bring original for your appointment)
- Student’s original (or certified copy) birth certificate or comparable legal document (also bring the original for your appointment)
- Student’s complete record of immunizations
- TWO (2) proofs of residency in the district in the parent/guardian’s name including ALL PAGES of current utility bill(s) for gas/electric/water/cable-internet/trash service, current lease agreement, or Deed/Settlement Statement from closing (see Pending Residency* or Residency by Friends or Family** below if you are unable to provide this information in your name)
- Special Education, Gifted, or Home-school placement information
- Finalized court documents allocating parental/guardian rights, as applicable
Students enrollment will not be completed until all documents are uploaded and reviewed by the registrar. Therefore, it is important to get all registration materials turned in as soon as possible. Thank you for your understanding and cooperation!
Please note: If a delay or closing occurs for SWL Schools due to inclement weather, appointments will be canceled accordingly. In the case of a delay, all appointments prior to 10 a.m. will be canceled. With a closing, all appointments for the day will be canceled. Notification will be sent to the email address provided on how to reschedule.
District Office Location and Directions
Our District Office is located at 927-A South Street, Pataskala, OH 43062. While enrollments are by appointment only, during the school year our general weekday office hours are 8:00am-4:00pm. Please note while school is out of session during the summer months, our office closes at 3:00pm.
Directions: From I-70
Traveling east, take Exit 118 – OH-310 Etna/Pataskala. At the traffic light, turn left. Once you travel through the first traffic light, turn right onto South Street and the District Office parking lot entrance will be to your immediate right before the tall brick school building. Our office is the tan administrative office building that faces west.

Preschool Registration
The SWL Early Learning center is a Preschool for Special Education (PSE), which means that we have a student population that consists of students with disabilities and peer models. We aim to individualize programming to each student throughout the year tailored at their own pace, so that their learning experience is focused on their developing needs and skills. Parents interested in applying for our Early Learning Center preschool can follow the steps below. Complete information about the ELC Preschool is available at our SWL Early Learning Center website.
Step One – Peer Model Application
Click HERE to complete the Peer Model application for new students that are 3+ years of age, potty trained, AND NOT currently on an IEP (Individualized Education Plan). If your student is currently on an IEP, please continue to Step Two.
Step Two - Online Pre-Registration
Click HERE to complete the online pre-registration from any device if your student RESIDES WITHIN the SWL Boundaries. Once you have created your account and completed all questions within the online form, a detailed checklist will be provided to you to download. If you have any questions about the pre-registration or checklist, please call 740-927-5437 during program hours.
Step Three – Enrollment Appointment
During the online registration you you will select an appointment date and time. Please bring your documents to the ELC Secretary at our Early Learning Center, located at 927-B South Street in Pataskala. Our Preschool is in the tall brick building that sits just north of District Office. You will enter through the yellow entrance on South Street with the yellow awning. Please allow 15-30 minutes of your time once we get started. Please be sure to have ALL documentation listed below and tuition payment (cash or check only) with you to complete enrollment or you may be asked to return for a second appointment.

School Assignment Boundaries
Resident students will be registered as follows:
- Preschool – Early Learning Center
- Grades KG - 3 – Kirkersville Elementary
- Grades KG - 3 – Pataskala Elementary
- Grades KG - 3 - Etna Elementary
- Grades 4-5 – Watkins Intermediate School
- Grades 6-8 – Watkins Middle School
- Grades 9-12 – Watkins Memorial High School
Pending Residency
If you are building or purchasing an existing home in the SWL district and you will not occupy this new residence prior to the enrollment of your new student(s), you may provide proof of your intended residency to register. When the House Under Construction Letter form and all required supplemental documents have been filed you will be permitted a maximum of NINETY (90) days in order to complete construction/closing and occupy your new residence to supply your established proof of residency. Residency with Friends or Family
If you are unable to provide proof of residency in your own name while currently living with family or friends that reside in the SWL District, you must submit a completed notarized Duplicate Residency Form, current photo ID, and two (2) current proofs of residency in the name of the person with whom you are residing. If you do not have the capability to print the form, please pick up or return the form to District Office Monday-Friday 8am-4pm. Click HERE to download, print, and complete the form.
Back to School Portal Annual Updates for Returning Students
Before the beginning of each school year, a legal guardian of students returning to SWL and entering grades 1-12 must complete our online Back To School Portal for the upcoming year through their Final Forms Account. This site simplifies the process of filling out forms for your children because much of the information you will be asked to complete will be automatically imported from our systems. Additional information you provide will then update your student’s records electronically with your digital signature.
You will need your Final Forms Login Information to complete this process. Students will be unable to pick up their schedules or receive classroom assignment information until this has been completed. Kindergarten parents DO NOT need to complete this.
Change of Address for Current Students
All requests for changes of address on school records should be completed by a legal guardian and submitted for processing. Changes in the actual physical address where the legal guardian and the student reside can be requested by completing the online Change of Address form, and by submitting current proofs of residency to swlreg@laca.org, as listed on the form. Click HERE to complete the online form and follow the prompts to submit the documentation needed.
Bus Transportation following Change of Address
If your student rides the bus to/from school, and you are requesting transportation change only not due to a physical address change, a legal guardian must submit an online Transportation Information form so bus routes can be changed. (Note: Any changes in the actual physical address will need to be authorized with the Change of Address process first, as described above.) Once completed, the transportation department will review the change request. All transportation changes require 2-3 working days to take place. ** Please note: Beginning FALL 2021 for grades K-3 ONLY, the alternate bus stop must be in the same attendance area as your child. Alternate stops outside that area will not be approved.
